Postprofessional Application

Graduate Certificate (Neurology, Orthopedics, Pediatrics)
A. Complete the online application.
B. Submit official transcripts of ALL postsecondary coursework taken at institutions other than the University of Indianapolis. We must be able to determine that you have an entry-level physical therapy degree.
C. Two letters of recommendation submitted using the online recommendation system. Immediately after you enter your recommenders' contact information in the online application, your recommenders are sent an E-mail asking them to submit a recommendation electronically. Select professional and academic references who can write knowledgeably about your potential within the program. Any paper recommendation forms should be submitted in a sealed envelope signed across the seal by the individual providing the reference.
D. Nonrefundable application fee of $50 paid when the online application is submitted.
E. Personal Statement should be entered in the appropriate section of the online application. In one or two pages, describe (1) how completion of this certificate will help you meet your professional goals, and (2) how your past achievements indicate your potential for success in our distance-format postprofessional program.
F. Copy of physical therapy license from the state in which you currently actively practice physical therapy. If you live outside of the U. S., then send a copy of the certification that allows you to practice physical therapy in your current region.
Note: No GRE is required for this program.

Postprofessional Master of Health Science

A. Complete the online application.

G. Resume should be downloaded in the appropriate section of the online application and detail your professional positions and accomplishments. We must be able to verify a minimum of 1,000 hours of clinical practice.

Retention of application materials. Application materials become the property of the University of Indianapolis and will not be returned to applicants. Application materials of unsuccessful candidates are retained for one year.
